New Patients & Forms

Your first visit to Bare Dermatology involves a few special steps so that we can get to know you. We ask that all new patients complete their paperwork prior to arriving for their first appointment.

New Patient Paperwork

All new patients will receive a text message and/or an email 3-5 days before your scheduled appointment date with a link to the paperwork that needs to be completed. If you are unable to complete this paperwork online via the link you are sent, you may download the paperwork using the “New Patient Paperwork” button below. If you are choosing to complete your paperwork by hand, please ensure you arrive 15 minutes before your scheduled appointment time in order for us to complete your new patient registration.
